https://docs.profoundinstitute.orgTechnical and operational documentation for Profound Health's Collaborative Care program (CoCM). Includes guides on EHR connectivity with Epic, AthenaOne, Allscripts (Veradigm), eClinicalWorks, NextGen, and others using FHIR and SOAP APIs. Covers security implementations via Azure Entra ID and Supabase RLS, billing models, partner onboarding, and integrations with Twilio, SendGrid, and pVerify.
What's inside Profound Health
- Profound Health integrates clinics, payers, communication channels, and AI services using deterministic APIs. The platform supports connectivity with EHRs such as Epic, Athena, and eCW utilizing SMART on FHIR, HL7, and custom connectors that map data to Supabase schemas.
Integrate NextGen EHR for patient data and care summaries
The NextGen integration supports both FHIR and proprietary APIs, prioritizing FHIR for portability. Key capabilities include: reading demographics, appointments, and clinical notes for CoCM patients; pushing care summaries as documents attached to the patient chart; and reconciling medication lists against Profound Health treatment plans.Understand the Notification Channel Architecture
The system uses anotifications-dispatcheredge function to route all outgoing communications. The providers used are Twilio Messaging Service for SMS, Twilio Programmable Voice for Voice calls, and SendGrid for Email. Templates are stored in Supabase.Integrate with eClinicalWorks (eCW) for patient data and scheduling
eClinicalWorks integrations utilize EHX FHIR and custom SOAP APIs, depending on the specific practice licensing. The integration primarily focuses on read access for continuity of care. Supported capabilities include:
- Importing patient demographics, encounters, and CCD documents.
- Receiving appointment schedules to trigger proactive outreach.
- Optional: Pushing visit summaries back to the system as signed PDF attachments.
Implement Oracle Health (Cerner) FHIR R4 functional capabilities
The Oracle Health (Cerner) integration utilizes FHIR R4 APIs with SMART on FHIR context. Supported functional capabilities include:
- Launching application context from PowerChart or Millennium via SMART on FHIR.
- Fetching
Observations,Conditions, andMedicationsresources for behavioral health reviews. - Creating
Taskrecords to notify Primary Care Providers (PCPs) of follow-up actions after a case review.
Understand Profound Health's HIPAA Regulatory Scope
Profound Health operates as a Business Associate (BA) for partner practices (Independent PCP practices, FQHCs, and health systems) delivering CoCM services.
Scope of Protected Health Information (PHI):
- Clinical notes
- Measurement results
- Medication lists
- Scheduling metadata synchronized via Supabase and external integrations.
Compliant Subcontractors: Downstream BAAs or HIPAA-eligible agreements are in place for LiveKit, Twilio, SendGrid, Stripe, pVerify, and analytic tooling.
Implement Epic SMART on FHIR workflows
Profound Health integrates with Epic using SMART on FHIR launch flows. The integration supports three primary workflows:
- Contextual Launch: Profound Health is launched within Epic, receiving patient and encounter context via the SMART payload.
- Clinical Data Sync: Pulls allergies, medications, problems, and lab results using Epic FHIR APIs (R4), normalizing them into the
care.fhir_resourcestable. - Task Write-back: Creates or updates
ServiceRequestresources for behavioral health follow-ups (requires partner contract enablement).
Implement the 'You Bill' billing model for CoCM
In the 'You Bill' model, the provider acts as the biller of record. The treating prescriber (PCP/NP/PA) performs the initiating visit and handles prescriptions. Profound Health provides the audit-ready minutes and artifacts required for the provider to submit claims using codes 99492, 99493, 99494, or G0512 (for FQHC/RHC). This model is the default implementation for CoCM.Understand the clinical scope of CoCM vs specialty therapy
Collaborative Care Model (CoCM) does not replace specialty therapy. Instead, it augments primary care by providing care management and consult psychiatry. Specialty care referrals are made as needed.Integrate with Elation REST and FHIR APIs
Elation provides REST and FHIR APIs that allow developers to perform the following actions:
- Retrieve patient demographics, vitals, and problem lists.
- Listen for encounter updates via webhook subscriptions.
- Post visit summaries and care plans as timeline entries.
Understand Profound Health's HIPAA and BAA compliance
Profound Health implements administrative, physical, and technical safeguards to protect patient interactions. They execute Business Associate Agreements (BAAs) with partners and maintain signed BAAs with all subcontractors that handle Protected Health Information (PHI).Integrate notifications and billing services
Profound Health provides the following specialized integration workflows:
- Notifications: Uses Twilio for SMS/voice and SendGrid for email templates, featuring delivery tracking and escalation policies.
- Eligibility & Billing: Uses pVerify for automated payer eligibility and Stripe webhook workflows for revenue capture.
